Register with the Practice
We are happy to accept patients who live in the villages as outlined below.
Our Practice area includes the villages of:
- Baddesley Clinton
- Shrewley Crossroads
Our practice area is defined by the Warwickshire County Border to the North, the A3400 to the West, the route of the M40 to the South and Ordnance Survey Grid reference SP 21 688 to the East. A map depicting the practice area is available in Reception.
You should contact the Surgery to establish that the address at which you wish to register falls within the Practice area. Our reception staff will be able to advise you on this.
Once you have established that you are living within the Practice area you will need to complete a form GMS1 for each member of the family together with a New Patient Questionnaire.

When you register we will invite you to make an appointment with our Practice Nurse for a new patient health check. If you are on regular medication you should make an appointment with one of the doctors as soon as possible.
If you are temporarily away from your normal place of residence and intend to stay in the practice area for more than 24 hours, but not more than 3 months, we will be happy to accept you as a temporary patient.
